Schedule an Examination

Once you have registered with DRS for the term, distributed your notification of disability memo to your instructor, and discussed taking your examination at DRS with your instructor, you may complete and submit this form to schedule an examination.

***Scheduling an Examination During Finals Week***

**Please note that DRS proctors exams between 8:00 a.m. and 7:00 p.m. during finals week. Please plan your exam times accordingly, as exams must be finished by 7:00 p.m.

**All final exams should be scheduled to begin on the hour.

**All final exams taken at DRS on Saturday, December 19 will begin at 8:00 a.m.

**On-line requests must be submitted at least 3 business days (72 hours) prior to the exam date.

Scheduling an Examination at 8:00 a.m. or after 3:30 p.m. During Finals

**To Schedule an exam during finals at 8:00 a.m. or after 3:30 p.m, please put the correct exam time in the "Description of Accommodations Notice" section, as the on-line scheduling system will not allow you to choose those times.

**Please be sure to select a time in the "Exam Time" section (even if you enter a time in the "Description of Accommodations Notice" section), or your request will not be submitted.

I M P O R T A N T: R E A D
Students arriving more than 30 minutes late for a scheduled examination will
not be permitted to sit for their exam.  Examinations will be immediately
marked as a NO SHOW and promptly returned to the instructor.

Exam Information

You must request exam accommodations at least three business days prior to the exam date. If you wish to arrange accommodations for an exam less than three business days in advance, please visit the DRS offices at 140 WPU. Exams must be taken during regular business hours. If you wish to schedule an exam for a class outside of business hours, such as a night class, please visit the DRS offices at 140 WPU.
